Preparing for an appointment involves a highly coordinated effort between the Administrative front office and the Clinical back office. The goal is to minimize "open chair time" (the time a room sits empty) while ensuring every patient feels like they are the only person in the building.
The preparation for an appointment starts well before the patient actually walks in, starting with the morning huddle, preparing consent forms depending on specific treatments, etc.
Specialty Checklists (Oral Surgery / Endodontics, etc…)
A structured specialty checklist ensures clinical readiness, reduces risk, and prevents last-minute delays for surgical and endodontic procedures. This is not just a quick review—it’s a proactive safeguard for patient safety, compliance, and efficiency.
Referral Verification: Referral Hub or Patient Documents
Confirm that the patient’s referral documentation is received, complete, and reviewed prior to the appointment. This includes ensuring the reason for referral, clinical notes, and any specific provider requests are clearly documented. Missing or incomplete referrals can delay treatment or require rescheduling.
Diagnostic Imaging (X-rays): Imaging Suite Articles (charting)
Verify that all necessary imaging is available, up-to-date, and diagnostically acceptable. This may include periapical, panoramic, or CBCT scans depending on the procedure. Ensure images are properly uploaded and accessible within the system for the provider to review before and during treatment.
Medical Clearance & Releases: Patient Documents
Ensure all required medical releases or physician clearances have been received, especially for surgical cases. Patients with complex medical histories (e.g., cardiac conditions, recent surgeries, or systemic diseases) must have documented approval before proceeding. This minimizes clinical risk and supports compliance.
Medication Review (Blood Thinners & Pre-Medication): Patient Alerts, Med Hx, patient Flags, Medical Alerts
Identify patients on blood thinners or other high-risk medications. Confirm whether modifications (pause/adjustment) have been approved by the patient’s physician and documented. Additionally, verify if pre-medication is required (e.g., antibiotics for certain conditions) and ensure the patient has followed instructions prior to arrival.
Case Readiness Confirmation: Work on the next upcoming DOS for patients
Confirm that all documentation, imaging, and approvals are completed at least one day prior to the appointment. This allows time to address any gaps and avoids chairside delays or cancellations.

Front Office Dashboard
The front office dashboard displays all appointments of the day, and allows you to filter out the appointments at a provider level.
The appointments are also categorized as All, Confirmed, Unconfirmed, Checked out and New patients, enabling you to reach out to the unconfirmed patients if needed, reviewing the confirmed appointments and preparing the paperwork for the new patients.
The appointments here can also be filtered out based on pending action items.

Patient Paperwork
Depending on the type of appointment and nature of the patient, the required information and consent forms might vary. CareStack gives you the ability to automate some of the workflows.
New Patient Forms |
Step 1:
| Navigate to Menu → Practice Settings → Office Wizard |
Step 2:
| Identify the form that needs to be set as a new patient form → Click the 3 dots → Click Edit |
Also click the settings button in the form editor.
Step 3:
Use the checkbox to select ‘Add to Patient Onboarding Form’
If this is a form that needs to be re-triggered at specific intervals, select ‘Trigger a new form every __ day(s) from the date of completion of the last form’ as well
Enter the suitable interval for re-triggering the form and click Save
Thus, when a new patient is created, CareStack will automatically add the form to the patient’s documents. The patient can either fill out the form using their patient portal or through the Kiosk at the time of Check-in. The same workflow can be applied to Financial policy, HIPAA consent form, etc as well.
Medical History Form
Similar to the new patient forms, medical history forms can also be set to trigger automatically when a new patient is created in the system.
Step 1:
| Navigate to System Menu → Practice Settings → Medical History Forms. |
Step 2:
| Select the Adult Questionnaire or Child Questionnaire tab as needed (the setup has to be done separately for each) and click Settings. |

| Click Edit on the bottom right of the Settings modal. |
Step 4:
Under Trigger Settings, you can:
Set the Trigger age for patients.
Create a form when a new patient is added: Select this option to create a new form for a new patient.
Trigger a new form '(enter the no of )' days from the date of completion of the last form (for adults). OR Trigger for patients below age 'n' (for child).
Click Save
General Forms
Any form that does not need to be added as a patient onboarding form as it might depend on the patient, but needs to be manually added to a patient depending on the situation can be added as follows:
Step 1:
