The latest update to our dental software features the addition of the ADA 2024 Claim Form. Here is what you need to know about the new form.
- Default Selection: Upon plan creation, the default selection for the ADA Claim form is set to 2019.
- Existing Plans: Existing plans will remain with the existing selection (ADA 2019 Form, ADA 2012 Form, ADA 2002 Form). At this time, there are no default selections at the location, carrier, or account levels. Should you wish to switch to the ADA 2024 Form for all plans, a bulk switch can be facilitated through a support request.
Box 53a., Locum Tenens Treating Dentist?: A new field has been added to accommodate services rendered by dentists in locum tenens (temporary substitute) status. This field requires manual input on the claim form directly before submitting it to insurance.
Box 39a., Date Last SRP: A new field has been added for recording the date of the patient's last Scaling and Root planning procedure. This field requires manual input on the claim form directly before submitting it to insurance.
Box 3a. and Box 11a. Payer ID: Two new fields have been added for recording the Payer ID for the Insurance Carrier (both Primary and Secondary). This field is filled automatically based on the Payer ID in the Carrier Address inside CareStack’s Carrier Practice Settings.
| Note: If any of the new fields have been utilized, this information WILL get transmitted to the Insurance Carrier for electronic claims on the 837D EDI as a remark, since there was no EDI update for the ADA 2024 Claim Form. |
