Create a New Referral Form
Explore this video for a thorough walkthrough.
Perform the steps below to get started.
- Navigate to the System Menu > select Practice Settings > Referrals > Referral Form Templates.
- Click + Add Referral Form.
A Create New Referral Form modal will appear,
- Enter the Referral Form Name.
- The Referral Form Name given by the user will be different from the Form Header that appears at the top of a form.
- Select a specialty to tag it to the referral form.
- Click Create.
A Referral Form will appear for the user to customize.
- Click Edit Header to update the header.
- Select the Practice Logo as Account Logo, Custom Logo, or No Logo.
- Change the Form Header name as needed.
- Click Save.
In the Patient Information section,
- Users have the option to enable or disable non-mandatory options such as Email and Parent/Guardian.
- Users can also enable or disable the questions in the referral form.
In the Referring Doctor Information section,
- Users have the option to enable or disable options such as Phone and Email.
- If Email is enabled, it becomes a mandatory field.
In the Tx Provider Information (Treatment Provider Information) section,
- Select the Preferred Tx Location (Preferred Treatment Location) from the dropdown.
- Users can enable or disable the Preferred Tx Provider option.
- Based on the Treatment Location selected, the Preferred Treatment Provider will vary.
- Click Edit Included Providers to update the list of providers.
- Select the desired providers.
- Click Save.
In the Procedures section,
- Users can enable or disable the Procedures or Others.
- By default, Extractions, Implants, and Full Arch are the procedures available.
- Click the delete icon to delete the given procedures.
- Click the edit icon to edit the given procedures.
- Click Add Procedures to add new custom procedures. Users can add up to 15 procedures.
In the Consultations section,
- Users can enable or disable the Consultations or Others.
- Click + Add Consultations to add new custom consultations. Users can add up to 15 consultations.
- Click the delete icon to delete the consultations.
- Click the edit icon to edit the consultations.
In the Teeth Numbers section,
- Users have the option to enable or disable non-mandatory options such as Teeth Selector and Enter Teeth Numbers.
In the Radiographs or Clinical Photos section,
- Users can enable or disable the Radiographs or Clinical Photos.
In the Add Case Notes section,
- Users can enable or disable the Add Case Notes.
After customizing the form,
- Click Preview to see the preview.
- Click Publish to publish the referral form.
- Once the form is created, you can view it under the Referral Form Templates section.
- Click Edit to edit the referral form.
To fill out the referral form,
- Click Copy Form Link.
- Open the link on any browser.
- Providers can use this link to refer a patient.
- Once the form is filled out, the referral will be added to the Referral Hub.
- Click Print Form to print the referral form.
- Practices can share it with external providers to refer patients.

To duplicate a referral form,
- Click the three-dot menu.
- Click Duplicate.
A Duplicate Referral Form modal will appear,
- Rename the Referral Form Name as needed.
- Change the Tag to Specialty as needed.
- Click Create.
- A copy of the selected form will appear,
- Make the required changes.
- Click Publish.
Manage Permissions

To enable Permissions,
- Navigate to the System Menu > select Practice Settings > Administration > Profiles.
- Click Manage Permissions for the intended profile.
- Select Practice Settings.
- Under Referral Settings, select the View Refer-In Form or Create/Edit Refer-In Form permissions, as needed.
- Click Save.
- On the Confirm Action modal that appears, click Yes to proceed.
- A toast notification appears stating, 'Profile updated successfully'.
