The Adult and Child Questionnaires in CareStack's medical history forms are sections that comprise both the Dental and Medical Questionnaire components. These questionnaires are designed to gather relevant health information from patients and are set up separately for adults and children, each with distinct Dental and Medical questionnaire sections.
Let's explore the steps to set up separate questions for each.
- Navigate to the System Menu > select Practice Settings > Medical History Forms.
- Select Adult Questionnaire or Child Questionnaire as needed.
- You can configure them separately.
- Select between the Dental or Medical Questionnaire.
- Choose the preferred Language to be English or Spanish.
- To edit the questions, click Edit.
If you want to hide the questionnaire from the medical history form, select the 'Do not show in Medical History Form' checkbox.
- To make changes to a question, click the Pencil icon.
- Update the change(s).
- To make a form component mandatory, check the option 'Make it Mandatory'.
- The mandatory form component will be marked with an asterisk in the medical history form.
- To allow users to select more than one option, check the option Enable Multi-Select.
- To highlight critical questions, check the Highlight when answered option and select the response that should trigger the highlight.
- This option is available for Single Choice, Multiple Choice, and Dropdown question types.
- Click Save.
- To add more Form Components, click and drag them from the right to the center. Fill in the form component > click Save.
- Click and drag the form component up or down to rearrange its order.
- Once the questionnaire is complete, click Save on the top right.
