When you merge a care note in a completed procedure that has been linked to that note, the selected notes will combine to form a single note, preserving the information from each note. Perform the steps below to add or merge care notes post code completion.
- Select the patient using the Global Search bar.
- Hover over Clinical > select Chart.
- Select the Code(s) that you would like to complete.
- Choose the Appointment date on the top left to filter out the related location, provider, etc as needed.
| Note: You will find the options for Appointment, DOS, Location, Provider, and Treating Dentist at the top for improved visibility and quicker access. |
- Right-click on the selected Code(s) and choose Complete from the menu
- To link to an appointment, select 'Yes' and choose the Appointment date.
- Click Complete to continue.
- Review and confirm the Provider and Fee in the warning modal.
- Select Proceed to continue.
On the Add Note slide-out, you will see notes that can be added.
Note: As shown in the example below, you can select and merge the composite care note. The amalgam note appears grayed out because it is mandatory and is automatically selected. |
Once you select the desired note, the Merge button is automatically enabled.
Note: Select Merge or Add as needed. Choosing Merge combines the notes into a single care note, while selecting Add keeps the notes separate. |
- As you proceed, the required criteria will already be selected before you click Finalize at the bottom right.

You can use the 'Delete' option to easily tackle instances wherein you accidentally created a care note in the wrong chart or prompted an incorrect note to be completed for the patient.
To delete the care note:
- Select the patient using the Global Search bar.
- Hover over Clinical > select Chart.
- Select or right-click the intended care note on the chart. Click 'Delete.'
- Select 'Yes' in the warning modal. Doing so will have the selected care note stricken off.
| Note: Finalized, mandatory, and locked notes will be stricken off. |
- A toast notification will appear on the top right stating, 'Care Note(s) deleted successfully.'
In an alternative instance, let's say you have started a note that you have not completed but want to delete it.
In such a case, navigate to the chart:
- Select or right-click the intended note under the 'To Start' section.
- Click Delete.
- Select Yes on the warning modal.
| Note: Since the note has neither been completed nor finalized, it will be deleted and not stricken off. |
Practices cannot edit a care note once it has been finalized. If you need to add some more information to a care note after it has been finalized, you can add an addendum. Perform the steps below to add an addendum.
- Select the patient using the Global Search bar.
- Hover over Clinical > select Chart.
- Click Notes to view all care notes of the patient > select the intended Finalized care note.
- Click Addendum at the bottom right.
- Select the Provider and enter the Addendum.
- Click Add.
- Once the Care Note addendum is added, you can view the assigned provider and the user who finalized the note.
- Once added, the details such as the date, time as well as the name of the user who had created the addendum will appear upon selecting, 'Show Details'.
